Maternal and Perinatal Risk Factors Associated with Vertical Transmission of Hiv-1 Infection from Mother to Child

چکیده

In 2009, the US Centers for Disease Control and Prevention (CDC) estimated that almost 11,000 children younger than 13 years old in the United States were diagnosed with HIV infection. Of those, it is estimated that the vast majority (88%) acquired the HIV infection perinatally. Given that both perinatal transmission is the most common route of HIV infection in children under 13 in the US and that childbirth rates among HIV infected women have increased by approximately 30% in recent years, we can expect additional cases of mother to child HIV transmission (vertical) unless e ective public health interventions are instituted. To do so, it is imperative to gain a better understanding of risk factors associated with vertical transmission of HIV-1 infection. We perform a secondary analysis of data from the Pediatric Pulmonary and Cardiovascular Complications of Vertically Transmitted HIV1 Infection Study to investigate these factors and to describe di erences in maternal and perinatal characteristics between HIV-1 transmitters and non-transmitters. We employed a bootstrap aggregation procedure using stepwise logistic regression to identify the potentially important predictors of increased HIV-1 transmission. Of the 17 variables we investigated, only CD4-CD8 ratio, vaginal bleeding during pregnancy, AZT used during pregnancy, and ruptured membranes greater than 24 hours were identi ed as signi cant risk factors. Date: December 5, 2014.

Maternal HLA homozygosity and mother-child HLA concordance increase the risk of vertical transmission of HIV-1.

BACKGROUND Mother-child human leukocyte antigen (HLA) concordance and maternal HLA homozygosity may increase the risk of vertical transmission of human immunodeficiency virus type 1 (HIV-1) risk by reducing infant immune responses. METHODS We analyzed mother-child HLA concordance and maternal HLA homozygosity in a Kenyan perinatal cohort receiving antenatal zidovudine. HLA concordance was sco...

Prevention of Mother to Child HIV Transmission

In Iran 8% of HIV- infected cases are women. Since most of them are young and in childbearing ages, in case of becoming pregnant and not receiving preventive measures, they can infect their child. Without Preventive measures transmission rate is 15 to 45 percent. Several factors such as sever stage of HIV and AIDS ,CD4 low count, high viral loud, acute retroviral phase, STDs, vaginal delivery, ...

Prevention of mother-to-child transmission of HIV in Africa.

HIV infection and mortality rates in African children are astoundingly high. Risk factors for mother-to-child transmission of HIV include maternal plasma viral load and breastfeeding. With regard to the latter, current data indicate that mixed feeding (breastfeeding with other oral foods and liquids) is associated with the greatest risk of transmission. Studies are under way to determine if exc...

HIV in Children in a General Population Sample in East Zimbabwe: Prevalence, Causes and Effects

BACKGROUND There are an estimated half-million children living with HIV in sub-Saharan Africa. The predominant source of infection is presumed to be perinatal mother-to-child transmission, but general population data about paediatric HIV are sparse. We characterise the epidemiology of HIV in children in sub-Saharan Africa by describing the prevalence, possible source of infection, and effects o...
